Required Qualifications

• Valid CT Technologist license in Illinois
• CT Technologist (1 year of experience)
• CT Technologist experience within the last year
• BLS issued by American Heart Association

Onboarding / Compliance Requirements

• COVID-19 vaccine documentation or approved declination/exemption, with declination not accepted for CA or IL
• MMR documentation
• Fit test for 3M N95 1860 respirator mask sizes small or regular
• Hepatitis B documentation or declination
• 2 Step PPD or IGRA (Quantiferon/Tspot)
• Varicella documentation
• Physical/Health Statement from an exam within the last year
• Trinity 10 Panel Drug Screen
• Trinity Profile: resume, skills checklist within a year, and 2 professional references
• Confidentiality of Patient and Proprietary Information and Information System Security Agreement
• Conflict of Interest Disclosure Form
• Criminal background check
