PT Home Health

Overview

• Travel/Contract role in Chesapeake, Virginia.
• Home Health Physical Therapist.
• Schedule: Day shift, 5x8 hours, 8:00 AM to 5:00 PM, Monday through Friday.
• Weekend coverage is every 4th or 5th weekend for PT.
• Mileage is reimbursed at IRS rate.
• 6 to 7 weighted visits per day.
• Contract length: 13 weeks.

Required Qualifications

• Valid Physical Therapist license in Virginia.
• Physical Therapist (1 year of experience).
• Physical Therapist experience within the last year.
• BLS - Basic Life Support issued by American Heart Association.
• CPR required.
• Will not consider candidates with any license or background hits.
• Fit test required; PAPR is not allowed.
• Negative 10-panel drug screen within 12 months of start.
• Proof of 2 references required.
• Complete application/resume required.
• Unit-specific skills checklist required.
• Physical exam within 12 months of start.
• Negative TB test or chest X-ray within 1 year of start.
• Hepatitis B titer within 10 years or declination.
• Varicella titer within 10 years.
• Rubeola titer within 10 years.
• Mumps titer within 10 years.
• Rubella titer within 10 years.
• Flu vaccination or approved exemption required by 10/31 during flu season.
• COVID vaccine preferred, but not required.
• Government-issued photo ID required.
• Vehicle registration form required.
• E-Verify required for direct placements.
• Fingerprinting required.

Preferred

• Home Health experience.
• MatrixCare or Epic experience.
• First-time travelers accepted.

Additional Notes

• Home Health and some other assigned groups require fully cleared fingerprint results before start.
• Housing or lodging is not mentioned.
