Hospital Pharmacist (Day/Evening Rotating Weekends) - Colorado

Location

  • Glenwood Springs, Colorado

Employment Type

  • Contract
  • Contract Length: 20 weeks

Schedule

  • Rotating day/evening shifts, 40 hours per week
  • Rotating weekends, 7:00 AM to 12:00 PM
  • Occasional 3rd shift (overnight) as needed

Job Summary

  • Prepare and dispense medication orders per physician requests
  • Maintain medication stock in patient care areas
  • Ensure safe, appropriate, and cost-effective medication therapies according to policies and protocols

Primary Duties and Responsibilities

  • Interpret medication orders and maintain computerized patient medication profiles
  • Compound and dispense pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition
  • Comply with federal, state, and local laws; manage controlled substances and maintain required records
  • Monitor drug therapy regimens for contraindications, interactions, allergies, and appropriateness of drug and dose
  • Perform pharmacokinetic drug dosing per hospital protocol
  • Review and interpret culture and sensitivity data for antibiotic appropriateness and recommend changes
  • Extract and interpret patient chart information accurately
  • Detect and report adverse drug reactions
  • Promote formulary adherence by minimizing non-formulary procurements and utilizing therapeutic substitution protocols
  • Provide clinical consultation and clarification to practitioners, suggesting cost-effective therapeutic alternatives
  • Provide drug information to patients and families as needed
  • Document all clinical activities and interventions accurately
  • Participate in quality improvement and medication review activities
  • Collect data, conduct quality monitors and inspections, and maintain logs and records
  • Assist in orientation, education, and training programs for pharmacy, nursing, and medical staff
  • Serve as a member of the Code Response Team

Required Qualifications

  • Valid Clinical Pharmacist license in Colorado
  • Minimum of 3 years of clinical pharmacist experience, including experience within the last year
  • BLS certification issued by the American Heart Association
  • ACLS certification
  • Experience with Epic electronic health record system

Additional Information

  • Traveler’s permanent address must be at least 75 miles away from the facility (radius rule)
