Case Manager

Job Overview

Travel/contract role in Chesapeake, Virginia
13-week assignment
Day shift: 5x8 hours, 8:00 AM–4:30 PM
Weekdays, with some weekends and holidays
No overtime without supervisor approval
Casual business attire

Required Qualifications

Valid RN license in Virginia
Associate of Science in Nursing / Associate Degree in Nursing
Acute Care Case Management (5 years of experience)
Acute Care Case Management experience within the last year
One completed travel assignment
BLS - Basic Life Support issued by American Heart Association
Experience working with Adult patients
Experience in an Acute Care setting
Discharge planning experience
Experience with Epic

Preferred Qualifications

CCM or ACM

Work Setting

Discharge planning on an ortho/neuro surgical unit
Fast-paced unit with multiple daily admissions and discharges
