• Port Angeles and Sequim, Washington

Contract

• 13 weeks
• Full-time, 0.9 FTE
• 36 hours per week

• Weekends and holidays

Required Qualifications

• Associate’s degree
• Current Washington State Registered Nurse license
• Current BLS certification
• Valid Washington driver’s license
• Proof of insurance on automobile used for work
• Reliable automobile transportation to travel to patient residences
• Driving record in compliance with employer standards
• Cell phone with activated voicemail
• Computer internet connectivity

Experience

• Minimum of 1 year recent experience in home health, acute care, or long-term care

Role Summary

• Provides nursing visits in patients’ homes as part of the interdisciplinary care team
• Supports continuity of care by carrying out delegated visits and communicating patient progress and physician orders to the case manager RN
