RN Case Management

Job Overview

• Employment type: Travel
• Location: Dover, New Jersey
• Schedule: Monday–Friday, 8:00 AM–4:30 PM
• Hours per week: 40
• Contract length: 13 weeks

Role Summary

• Coordinates patient care across the continuum through collaboration with the interdisciplinary care transitions team.
• Supports assessment, care planning, plan implementation, evaluation, utilization review, discharge planning, and continuity of care.

Required Qualifications

• Active New Jersey RN license or multi-state RN license
• BLS certification through the American Heart Association
• 4 years of recent RN case management experience
• Experience in an LTAC or STACH setting
• Experience with MCG
• Experience with discharge planning across acute care settings
• Strong interdisciplinary team collaboration experience
