Case Manager (Registered Nurse) - Travel/Contract

Location: West Islip, New York
Employment Type: Travel/Contract
Shift: Day (3 shifts per week, 12 hours each; 7:00 AM - 7:30 PM)
Hours per Week: 36 hours
Contract Length: 13 weeks

Job Summary

• Manage care coordination as a Registered Nurse, focusing on safe, quality patient progression from admission to discharge.
• Collaborate with multidisciplinary teams to optimize patient experience and bed assignments.
• Lead unit meetings to identify and remove barriers to patient care plans.
• Participate in escalation huddles to address discharge barriers with key stakeholders.
• Serve as a clinical and technical resource and role model for staff.

Required Qualifications

• Valid Registered Nurse (RN) license in New York.
• Bachelor of Science in Nursing (BSN) degree.
• Minimum 1 year of case management experience within the last year.
• Experience in manager roles.
• Basic Life Support (BLS) certification issued by the American Heart Association.

Compliance and Documentation Requirements

• Valid driver’s license or state ID with clear picture and current address.
• Background checks including:

  • 7-year background check completed within 30 days of start and updated annually.
  • National Sex Offender, National Criminal, FACIS Level 3, OFAC, OMIG (NY Medicaid exclusions), OIG, SAM checks.
• Employment application covering 7 years of employment history with explanations for any gaps.
• Skills checklist current within the last year and applicable to the position.
• New York State Medicaid Exclusion List check updated annually.

Health and Safety Requirements

• Facility-specific physical exam completed by a physician within 30 days prior to start, using the required form. Must include candidate name, medical history, physical exam, physician signature, and credentials.
• Fit test for N95 mask (Halyard Fluidshield or 3M 1860) completed within 12 months prior to start and updated annually.
• Proof of negative TB test (QGold, TSpot, or PPD) within 1 year of start; if positive, clear chest x-ray within past 12 months and completed questionnaire required.
• Immunizations or positive IgG titers required for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Varicella
• Hepatitis B antibody and antigen results required; proof of vaccine series alone is insufficient. Option to decline or revaccinate if antibody negative.
• Seasonal flu vaccination required October 1 - April 30 (declinations allowed for religious or medical reasons).
• COVID vaccination series preferred.
• Standard 9-panel drug screen required within 30 days prior to start and updated annually; rapid drug tests not accepted.

Competency Requirements

• Medication Test/Pharmacology Competency Exam with minimum score of 80%.
