Job Overview

• Schedule notes: May include occasional weekend coverage and floating

Required Qualifications

• Valid Speech Language Pathologist license in California
• 1 year of Speech Language Pathologist experience
• Speech Language Pathologist experience within the last year
• Experience in a Long Term Care setting
• BLS issued by American Heart Association or American Red Cross

Compliance / Onboarding

• Government-issued photo ID
• Emergency contact form before start
• Standard background check
• EPLS/GSA/SAM screening
• HHS/OIG screening
• CA wage and rehab hour quiz
• Physical exam within 1 year of start
• Negative TB test or chest X-ray within 1 year of start
• Measles, mumps, and rubella documentation based on birth year requirements
• Hepatitis B documentation
• Varicella documentation
• Drug screen within 1 year of start
• Flu vaccination or declination during flu season
• 2 professional references within 3 years of start
• Annual skills checklist

Certifications

• ACLS
• PALS preferred
