Care Manager

Location: Bronx, New York
Employment Type: Acute - Allied
Contract Length: 14 weeks

Job Summary

Position is for Inpatient (IP) care, not Emergency Department (ED).
Full-time schedule: 7 hours per day, 35 hours per week.

Required Qualifications

Valid RN license in New York
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN) from an accredited program
Minimum 5 years of experience in Acute Care Case Management, including experience within the last year
Experience as a Care Manager
Experience with Care Coordination and Utilization Review
Experience with Discharge Planning
Basic Life Support (BLS) certification
PRI and Screen Certification

Preferred Qualifications

Bachelor’s or Master’s degree in Nursing.
Leadership experience.
Case Management Certification.

Additional Information

Candidate must have PRI and Screen certification at the time of submission.
