Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF)

Required Qualifications

Valid Occupational Therapist license in Hawaii
Occupational Therapist (2 years of experience)
Occupational Therapist experience within the last year
BLS

Schedule

Day shift
Generally 5 days per week, with likely one weekend day
May require floating to other units or facilities based on census and staffing needs
Must be available for Hawaii state and federal holidays as needed

Responsibilities

Provide rehabilitation services
Perform evaluations, reassessments, treatments, and other clinical services within scope of practice
Develop, implement, or carry out individualized treatment plans
Provide evidence-based therapeutic interventions
Educate residents, families, and caregivers
Complete timely and compliant documentation
Supervise therapy assistants when applicable

Training / Onboarding

Orientation provided for the rehabilitation department, documentation workflow, and facility workflows

Start Date Requirement

Must arrive the Thursday before start for an employee health appointment on Friday
