- Skilled Nursing Facility (SNF) and/or Intermediate Care Facility (ICF)
Required Qualifications
- Valid Occupational Therapist license in Hawaii
- Occupational Therapist (2 years of experience)
- Occupational Therapist experience within the last year
- BLS
Schedule
- Day shift
- Generally 5 days per week, with likely one weekend day
- May require floating to other units or facilities based on census and staffing needs
- Must be available for Hawaii state and federal holidays as needed
Responsibilities
- Provide rehabilitation services
- Perform evaluations, reassessments, treatments, and other clinical services within scope of practice
- Develop, implement, or carry out individualized treatment plans
- Provide evidence-based therapeutic interventions
- Educate residents, families, and caregivers
- Complete timely and compliant documentation
- Supervise therapy assistants when applicable
Training / Onboarding
- Orientation provided for the rehabilitation department, documentation workflow, and facility workflows
Start Date Requirement
- Must arrive the Thursday before start for an employee health appointment on Friday
