Job Opportunity: RN Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Job Summary

As a Home Health RN, you will:

Assess patient conditions
Implement care plans
Provide ongoing education and support to patients and their families
Perform admission processes and case management responsibilities
Deliver patient care in accordance with organizational philosophy, policy, and community health practice standards
Utilize thorough and timely electronic documentation and interdisciplinary communications
Travel to cases as assigned
Participate in weekend rotations once every 6 weeks

Experience Requirements

Minimum 1 year Certified Home Care experience
Minimum 1 year Case Management experience

No prior travel experience is explicitly required.

Credentials & Compliance Requirements

Professional nursing license verification (Primary Source Verification required; MA Conditional Licenses not accepted)
Submission of a recent color photo (within past 6 months) meeting specific guidelines
Motor Vehicle Record (MVR) check within 45 days of start
Criminal background checks including Massachusetts CORI, National Sex Offender Check, and Exclusion Checks (OIG, GSA, Medicaid lists)
Resume including past 7 years of work experience with city & state
Two recent references (minimum one supervisor or above) in the skillset
Skills checklist required for submission
Confirmation of assignment form upon acceptance and for extensions

Health & Safety Requirements

Fit Test for N95 respirator within 365 days of start, updated annually
Respirator Medical Clearance Attestation (if fit test required)
Documentation of immunizations or immunity for:
- MMR (Measles, Mumps, Rubella)
- Influenza vaccine during flu season (Sept 15 - April 30) with no exemptions
- TB test (skin test or blood test) within 3 months of start, updated if >6 month break
- Health statement signed by MD/DO/NP/PA within 365 days of start, updated if >6 month break
Hepatitis B vaccine strongly preferred
Tdap vaccine preferred within last 10 years
Additional TB-related documentation if positive test results

Additional Notes

Ability to travel to patient cases as assigned is required
Weekend rotations occur once every 6 weeks

This position is ideal for experienced Home Health RNs with case management skills who are comfortable with travel and comprehensive patient care in a community health setting.
