Job info

Provides skilled nursing and case management for patients in their place of residence
May start as a Visiting RN and transition to Case Manager
Coordinates care, supports continuity of care, and uses the nursing process in an interdisciplinary team setting

2 years of recent acute care, home care, or hospice experience
Experience with venipuncture, catheterization, wound care, and/or parenteral therapy
HCHB experience

BSN preferred
Maintains patient records in accordance with agency standards and OASIS requirements
