Unit & Shift Details

• Unit: Intermediate Care Unit (Telemetry)
• Shift: Day shift, 3 shifts per week, 12 hours each (7:00 AM - 7:30 PM)
• Hours per week: 36
• Nurse-to-patient ratio: 1:5-6
• Weekend requirement: Every other weekend
• Floating: Floating within scope to meet facility needs is required
• Block scheduling: Not accommodated
• Local candidates within 75 miles radius are not accepted

Experience & Requirements

• Valid Registered Nurse (RN) license in Illinois
• Minimum of 1 year of telemetry nursing experience within the last year
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• ACLS (Advanced Cardiac Life Support) certification
• BLS (Basic Life Support) certification issued by the American Heart Association
• NIH Stroke Scale (NIHSS) certification

Certifications Required

• ACLS (Advanced Cardiac Life Support) - AHA only; certs of completion acceptable for up to 1 month until official card is issued
• BLS (Basic Life Support) - AHA only; certs of completion acceptable for up to 1 month
• NIH Stroke Certification (annual update required unless 2-year expiration listed)
• Additional certifications may be required based on unit-specific job descriptions (e.g., CPI, MAB, PALS, NRP, TNCC, EFM) — verify with unit details

Licensure

• Active Registered Nurse license for Illinois (no pending licenses accepted)
• Primary source verification of professional license required
• Illinois temporary licenses require approval and must convert to permanent for extensions

Compliance & Background Checks

• Criminal background check within 6 months, including statewide and nationwide searches
• Quarterly EPLS/GSA/SAM and HHS/OIG checks
• Illinois fingerprinting required for assignments at or floating to long-term care or rehab units
• Education verification of highest nursing degree required prior to start
• State-specific exclusion and sanction list checks for Illinois placements
• Drug testing within 1 year of start, including a comprehensive panel; must remain valid throughout assignment

Health & Immunization Requirements

• Physical exam within 1 year of start
• Fit mask testing (N95 respirators) required and must be renewed annually
• Tdap vaccine or declination (expires every 10 years)
• Negative TB test or chest x-ray per protocol; yearly TB questionnaire required
• Measles, Mumps, Rubella, Varicella: Positive titer, vaccination, or medical exemption required; no exemptions allowed for certain units (e.g., Neonatal ICU)
• Seasonal flu vaccination or medical/religious exemption required during flu season (Sept 30 - Apr 1)
• Hepatitis B declination required (vaccination not mandatory)

Additional Required Documents & Training

• Employment application
• Two professional references (supervisors preferred; specific requirements for RNFAs)
• Unit-specific skills checklist within 1 year of start (Oncology skills checklist required for Med Surg-Oncology Unit)
• Annual competency exams, including medication/pharmacology and unit-specific exams
• Facility-specific documents including HIPAA acknowledgement, confidentiality form, latex allergy questionnaire, hand hygiene, fingernail, and dress code policies
• Regulatory compliance education and orientation modules completed annually via online platform prior to start
• Badge photo (head and shoulders, white background, no obstructions, taken within 6 months)
