Location: Fresno, California
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Details

• Schedule: Monday to Friday, 8:30 AM – 5:00 PM (30-minute lunch)
• Average Patients per Day: 5-7
• Case Type: Home Health – Geriatric population
• Territory: 50-mile radius from office/hub
• Electronic Medical Record (EMR): Experience required with HCHB
• OASIS experience required
• Mileage reimbursed at IRS standard rate between patient homes
• Locals accepted at travel rate

Required Qualifications

• Valid Physical Therapist license in California
• Minimum 1 year of Physical Therapist experience within the last year
• Experience in a Home Health setting
• Experience with Homecare Homebase (HCHB)
• BLS certification required (Basic Life Support)

Required Documentation & Compliance

• Government-issued photo ID (Driver’s License or State ID)
• Emergency Contact Form prior to start
• Background checks including 7-year residence/work/school history, sex offender registry, EPLS/GSA/SAM, and HHS/OIG (updated annually)
• Certifications: BLS required; ACLS and PALS preferred but not mandatory
• State License verification via primary source
• Competency exams as applicable (Medication/Pharmacology and general competency)
• Physical exam within 1 year of start date
• Immunizations and titers as applicable:

  • Measles (Rubeola), Mumps, Rubella (for candidates born 1957 or later)
  • Hepatitis B (titer, declination, or vaccine series)
  • Varicella (titer, vaccine, or history of chickenpox)
  • Seasonal flu vaccination or declination during flu season (Oct 1 – Mar 31)
• Drug screening: 9-panel drug screen within 1 year of start (instant drug screen acceptable)
• References: 2 professional references required for travel/allied/per diem acute care roles (within 3 years)
• Annual skills checklist required

Additional Information

• No weekend or on-call shifts required
• First-time travel Home Health Physical Therapists are not accepted for this role
• Must have experience with Home Health documentation and processes (OASIS and HCHB EMR)

This position is ideal for a Physical Therapist with at least one year of home health experience, comfortable working independently in a geriatric home health setting, and licensed in California.
