Overview
- Location: Newton, Massachusetts
- Employment type: LTA
- Contract length: 13 weeks
- Shift: 08:00 - 16:30
- Weekend rotations are once every 6 weeks
- Ability to travel to cases as assigned
Required Qualifications
- Valid RN license in Massachusetts
- Associate of Science in Nursing / Associate Degree in Nursing
- Home Health experience within the last year
- 1 year of Certified Home Care experience
- Case Management experience
- License/Certificate Verification is required

- Assess patient conditions
- Implement care plans
- Provide ongoing education and support to patients and their families
- Perform admission processes
- Perform case management responsibilities
