Job Description

  • Position in Cardiac ICU within a well-known hospital system (Ascension Health)
  • Role requires specialized ICU nursing skills

Required Credentials & Onboarding Requirements

Submission Requirements (Must be provided before submission)

  • Current Nursing License and Primary Source Verification (within 30 days prior to start)
  • Nursys license verification (within 30 days prior to start)
  • Professional Resume/Profile
  • At least two professional references
  • Valid Driver’s License or State ID (must not be expired and must meet address requirements)

Onboarding Requirements (Must be completed before or at start)

  • Ascension Cerner Test-Out (Cerner test results and training checklist with signed attestation)
  • Ascension Badge Photo (forward-facing headshot with white or blue background, no alterations)
  • Health and Immunization Documentation:
    • MMR: Proof of two vaccines or positive IGG titer (no declinations accepted)
    • Varicella: Proof of two vaccines, immunity, or history verified by healthcare provider (no declinations accepted)
    • Hepatitis B: Vaccination series completion or signed declination
    • Influenza Vaccine: Optional, but if declined during flu season (Oct 1 - Mar 31), mask required
    • Tdap: Valid within 10 years, no declinations accepted
    • 2 Step PPD or IGRA testing with annual TB questionnaire
    • Positive PPD requires negative chest X-ray within 1 year and annual TB questionnaire
  • Fit Test: Required for all travelers, preferred before start (specific mask types required)
  • Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually
  • Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually
  • Criminal Background Check: Includes SSN trace, employment verification, OIG search, national sex offender check, updated annually
  • Employment Verification: Past 7 years, updated annually
  • Education Verification: Relevant to clinical work, completed within 90 days of start
  • Ascension Skills Checklist: Clinical skills checklist completed within 1 year, updated annually
  • Ascension Specialty Exam: Minimum 80% passing score, updated every 3 years
  • Ascension Core Competencies: HIPAA, Joint Commission, OSHA topics, minimum 80% score, updated yearly
  • Ascension EKG Exam: Minimum 80% passing score, updated as required
  • Ascension Color Vision Test: Completed prior to start, updated every 3 years
  • BLS Certification (AHA)
  • ACLS Certification (AHA)
  • NIH Certification: Group A yearly, Groups B-F every 2 years
  • CPI/MOAB High Risk Online Portion: Completed prior to start (Nonviolent Crisis Intervention or MOAB De-Escalation Course)
  • Ascension Onboarding Education Attestation: Completion of pre-hire training modules
  • Ascension Nursing Contract Worker Onboarding: Completed onsite
  • Ascension Nova StatStrip Pre-Hire Modules: Required for non-allied positions

Optional Credentials

  • Influenza Vaccine Exemption/Declination (mask required if declined)
  • Positive PPD documentation (if applicable)
  • Ascension Unit Specific Checklist (optional, uploaded by unit leader)
  • Hiring Manager Evaluation (post-assignment)
