Required Qualifications

• Valid Physical Therapist license in Hawaii
• Physical Therapist (2 years of experience)
• Physical Therapist experience within the last year
• Bachelor's degree
• Experience in a(n) Home Health setting
• Driver's License
• Experience with Epic

Preferred Qualifications

• Master’s degree
• Travel experience

Job Responsibilities

• Plan and conduct treatment programs to restore and improve physical, social, and mental functions
• Complete evaluations and address areas needing attention
• Develop individualized treatment and discharge plans
• Provide therapy interventions using appropriate protocols and techniques
• Complete accurate clinical documentation within established deadlines
• Supervise assigned assistants
• Serve as co-leader in an interdisciplinary approach to patient groups
