Registered Nurse (RN) - Medical/Surgical

Location: Ithaca, New York
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: Night (18:45 - 07:15)


Job Summary

  • Provide direct patient and family support and nursing care within full scope of practice.
  • Participate in development, implementation, monitoring, and evaluation of patient and community programs and services.
  • Responsible for observation, data collection, planning, implementation, and evaluation of patients’ physical, psychological, emotional, educational, and spiritual needs based on individualized care plans.
  • Collaborate as a member of the interdisciplinary team.
  • Block scheduling is not guaranteed but accommodations will be attempted when possible.

Required Qualifications

Licensure & Certifications

  • Valid RN license in New York.
  • Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross, must remain valid throughout assignment.

Experience

  • Minimum of 2 years of Medical/Surgical nursing experience.
  • Medical/Surgical nursing experience within the last year.

Education

  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).

Exams & Documentation

  • Completion of Centralus Core Competency Exam.
  • Rhythm Interpretation Exam.
  • Valid U.S. driver’s license.

Required Onboarding Documentation & Compliance

Professional Requirements

  • Completed skills checklist for the assigned unit, updated annually.
  • Core Competency Exams (Nursing Core Comp I, II, and III) with a minimum score of 80%, updated annually.
  • Rhythm Interpretation Exam.
  • Valid U.S. driver’s license or state ID (unexpired).
  • Agency background check within 30 days of start, including:
    • SSN address trace report covering all counties lived/worked in last 7 years.
    • FACIS 3 check.
    • County and national criminal searches.
  • Centralus resume documenting all employment and gaps for the last 7 years, with education history.
  • Two professional references:
    • One supervisor reference from employment within the last 12 months.
    • One supervisor or peer reference from employment within the last 3 years.
  • Employment verification of most recent employer (agency verification accepted).
  • Signed and dated job description.
  • Completion of required agency forms including:
    • Personal Information Form
    • Code of Conduct
    • Confirmation of Assignment Form
    • Self Study Packet Documentation and Quiz
    • Sexual Harassment Acknowledgement
    • Parking Registration Form
  • Completion of 7 instant background checks (manual upload required).
  • AH Drug Tobacco Acknowledgement.
  • Pharmacology Exam.
  • Centralus Epic Questionnaire (submission required).

Health & Immunization Requirements

  • COVID-19 vaccine proof or agency declination (optional).
  • MMR: Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination series must be completed within 30 days of start.
  • Fit Test: Must be completed within last 12 months prior to start; annual requirement.
  • Hepatitis B: Positive antibody titer, 3 vaccine series, or specific declination form required.
  • TB Test: One of the following within last 12 months:
    • Two skin PPDs (one within 12 months, one within 3 months), or
    • One Quantiferon/Spot test.
    • For past positives, chest x-ray within 12 months may be required.
  • Varicella: Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination series must be completed within 30 days of start.
  • Tdap: Within last 10 years or specific declination form.
  • Physical/Health Statement: Within past 12 months, including clearance with no restrictions, vital signs, height, weight, Snellen eye chart, and clinician details.
