Oncology Clinic RN Travel Contract

Location

• Lakeport, California

Schedule

• Day shift
• 4x10 hours
• 7:00 AM to 5:00 PM
• 40 hours per week

Contract Length

• 13 weeks

Setting

• Outpatient infusion center

Required Qualifications

• Valid California RN license
• Associate of Science in Nursing / Associate Degree in Nursing
• 2 years of oncology experience
• Oncology experience within the last year
• ACLS certification issued by the American Heart Association or American Red Cross
• BLS certification issued by the American Heart Association or American Red Cross
• PALS certification issued by the American Heart Association or American Red Cross
• ONS/ONCC certification

Additional Requirements

• 7-year background check within 6 months of start
• FACIS Level 3 screening within 2 weeks of start and monthly after start
• EPLS/GSA/SAM screening
• HHS/OIG screening
• Sex offender search within 90 days of start
• California Medi-Cal search if FACIS Level 3 cannot be run monthly
• Education verification of highest level achieved, unless highest education is high school
• Official transcripts or diploma may be accepted only if verification attempts are unsuccessful
• Full 7 years of work history in the application, with city and state for each location and no gaps
• Work history verification for up to 7 years, with gaps over 30 days explained
• Unit-specific skills checklist
• Employment application review/sign prior to start
• Standards of business conduct review/sign prior to start
• Job description review/sign prior to start
• Conditional offer of employment attestation prior to start
• General regulatory orientation review/sign prior to start
• Vendor training, code of conduct, and overpayment refund policy review/sign prior to start
• TB screening questionnaire within 90 days of start
• Negative TB test or chest x-ray within 90 days of start
• Physical exam within 1 year of start
• Fit test within 1 year of start
• Respiratory safety clearance prior to start
• Drug screen within 6 months of start
• Tdap vaccination within 10 years of start
• Measles, mumps, rubella, varicella, and hepatitis B immunity or vaccination documentation
• Flu vaccination documentation during flu season
• Color vision screening by a licensed healthcare provider if applicable
• References are required by the staffing agency, not the client
