Required Qualifications

Valid Pharmacist license in Oregon
Minimum of 1 year of pharmacist experience
Pharmacist experience within the last year
Experience in an outpatient setting

Background and Compliance Requirements

Government-issued photo ID (Driver’s License or State ID)
Social Security Card (as applicable)
Background Check within 90 days covering all locations lived/worked in past 7 years; must be updated annually
EPLS/GSA/SAM and HHS/OIG checks within 30 days of start, updated monthly
Education Verification required
Drug Testing: 9 Panel Drug Screen within 12 months of start (instant acceptable)
Physical Exam within 1 year of start, signed by MD, NP, PA, or DO
Immunizations and Health Screenings:
- TDAP vaccine within 10 years (declination accepted)
- Negative TB test or chest X-ray within 12 months (annual updates required)
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal Flu vaccine during flu season (declination accepted)
- COVID vaccination record preferred (declinations not required)
Respiratory Safety Clearance (RespSafety) required annually for travel candidates
Photo for badge (passport style with neutral background) required

References and Employment History

Minimum of 2 references covering 1 year of employment within the last 3 years in a comparable specialty/setting
Employment application covering at least the last 5 years of employment with month/year format for dates
Work history verification may be required if reference requirements are not met

No prior travel experience explicitly required
No on-call or weekend shifts required
Float policy acknowledgment may be required (traveler may be asked to float to other nearby facilities)
Epic training modules required to be completed within a week of orientation
Annual regulatory and specialty-specific training modules required via LMS (Workramp)
Facility uses Epic for charting and documentation
