Job Opportunity: RN Case Manager

Location: Fort Wayne, Indiana
Contract Length: 13 weeks


Job Description

Seeking an experienced RN Case Manager for a travel assignment in a hospital setting. The role focuses on inpatient utilization management, working collaboratively with the care team to drive patient discharge and stay progression.


Key Responsibilities

  • Conduct comprehensive patient assessments to determine patient status and facilitate discharge planning.
  • Drive stay progression and expedite patient transitions to the next level of care.
  • Complete accurate and timely documentation using Epic electronic health records.
  • Collaborate with the care team and participate in utilization review activities.
  • Primarily assigned to one inpatient unit but may float to other units while performing the same case management duties.

Schedule

  • 8-hour day shifts, Monday through Friday.
  • Participation in weekend rotation required.

Unit Details

  • Units range from 20 to 36 beds.
  • One or two RN Case Managers per unit depending on size.

Required Qualifications

  • Valid RN license in Indiana
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
  • Minimum 2 years of case management experience, including experience within the last year
  • Experience in an inpatient setting
  • Utilization management and utilization review experience, including proficiency with InterQual
  • BLS certification

Skills and Abilities

  • Ability to work independently and identify necessary tasks without prompting.
  • Strong collaboration skills with multidisciplinary care teams.
  • Proficient in comprehensive patient assessment and discharge planning.
  • Accurate and comprehensive documentation skills.

Orientation and Training

  • General hospital orientation (6 hours).
  • Epic training (4 hours).
  • Role-specific training with a peer (8-16 hours).
  • Limited assignment with peer oversight (8-16 hours).

Dress Code

  • Business casual with white lab coat (no jeans) and close-toed shoes,
    or eggplant-colored scrubs (top and bottom).

Additional Notes

  • Local clinicians within 50 miles may be eligible for a local rate adjustment.
  • The client may cancel or call off up to 24 hours before the start of the assignment.
  • Orientation includes 8 non-billable hours; clinicians returning within 12 months are not eligible for non-billable orientation.
