Job Opportunity: Step-Down Registered Nurse (RN)
Location: Auburn, New York
Employment Type: Travel/Contract
Shift: Night (3 shifts per week, 12 hours each; 7:00 PM – 7:30 AM)
Guaranteed Hours: 36 hours per week
Contract Length: 13 weeks
Unit & Patient Population
- Step-Down unit with patient ratio approximately 1:4-5
- Common diagnoses:
- Congestive Heart Failure (CHF)
- Post heart catheterization
- Post vascular procedures
- Respiratory failure requiring BiPAP, CPAP, or Vapotherm
- Cardiac dysrhythmias
Clinical Responsibilities & Procedures
- Post-heart catheterization care including TR band management
- Monitoring after vascular procedures
- Frequent vital signs and neurovascular checks
- Cardiac monitoring and dysrhythmia interpretation/management
- 12-lead EKG/ECG interpretation
- IV therapy including IV starts, PICC line management, and blood product administration
- Administration and monitoring of emergency medications and procedural sedation
- Cardiac drip titration
- Management of respiratory support devices (BiPAP/CPAP)
- Assistance with chest tube insertion and removal
- Stroke (CVA) and acute stroke management
Required Certifications
- Basic Life Support (BLS) – AHA
- Advanced Cardiac Life Support (ACLS) – AHA
- NIH Stroke Scale (NIHSS) certification
Requirements/Qualifications
- Valid Registered Nurse (RN) license in New York State
- Minimum 2 years of Step-Down nursing experience, with experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Current Basic Life Support (BLS) certification issued by the American Heart Association
- Advanced Cardiac Life Support (ACLS) certification
- NIH Stroke Scale (NIHSS) certification
- Proficient in cardiac monitoring, dysrhythmia interpretation, and dysrhythmia management
- Experience with PICC line management
- Skilled in administration and monitoring of procedural sedation
- Competent in cardiac drip titration
- Knowledge and experience in management of cerebrovascular accident (CVA) and acute stroke patients
Licensure
- Primary source license verification required prior to start
Background & Compliance Requirements
- Background check within 1 year covering all locations lived/worked/schooled in past 7 years (complete results required)
- EPLS/GSA/SAM and HHS/OIG checks prior to start
- Nursys verification prior to start
- Review and sign facility-specific documents including Code of Conduct, Patient Abuse Reporting, Confidentiality, Orientation Packet, and Automated Dispensing Cabinet Security (for Pyxis access)
- Annual Tuberculosis screening questionnaire and negative TB test or chest x-ray as applicable
- Physical exam within 12 months prior to start, signed by licensed provider confirming ability to perform job duties
- Fit mask test within 12 months, updated annually (specific approved mask types required)
- Immunizations/vaccinations or documented immunity required:
- TDAP (within last 10 years or declination)
- Measles, Mumps, Rubella (2 MMR vaccines or serologic titers)
- Hepatitis B (series, titer, or declination)
- Varicella (2 vaccines, positive titer, or documented history)
- Seasonal flu vaccine or declination during flu season
- COVID vaccine records or declination
- 9-panel drug screen within 30 days of start (instant/rapid accepted)
Additional Requirements
- Two professional references within 3 years of hire
- Skills checklist completion required
- New York State Nurse Aide Registry verification required
- New York State Medicaid Exclusion List check required
Notes
- Local candidates within 50 miles are accepted (with adjusted pay rate)
- Medication system used: Pyxis automated dispensing cabinet
- No blocked scheduling; every other weekend (EOW) and every other holiday (EOH) schedule
