Overview

Travel/contract role in Portland, Oregon
Home health setting
Start date: ASAP
Contract length: 13 weeks
Shift: Day, 5x8 hours, 8:00–16:30
First-time travelers accepted
Local candidates accepted
No weekend requirement
No on-call requirement
No floating between like units
Block scheduling not available

Required Qualifications

Valid LMSW - Licensed Master Social Worker license in Oregon
LMSW - Licensed Master Social Worker (1 year of experience)
LMSW - Licensed Master Social Worker experience within the last year
BLS issued by American Heart Association or Heart and Stroke Foundation
Experience in a Home Health setting
Experience with OASIS
Own transportation
Primary series COVID vaccination plus one booster

Schedule and Workload

8-hour day shifts
Facility cannot accommodate 4x10 or 3x12 schedules
May visit up to 6 patients per day
Standard productivity: 30 points
Occasional daily travel radius of roughly 60 miles, with 60–90 minutes total drive time

Assignment Details

Home health social work role
Patient ratio: 1:1
Pending license accepted if Oregon licensure is in progress
Local candidates are considered within a 50–75 mile radius

Additional Notes

Candidate must have their own transportation
RTO restrictions to be confirmed with hiring manager
Facility may call off up to one shift per pay period
Business casual attire; scrubs allowed but not required
Free parking
Mileage reimbursed at IRS standard rate
