Required Qualifications

• Valid Physical Therapist license in Massachusetts
• Physical Therapist (2 years of experience)
• Physical Therapist experience within the last year
• BLS issued by American Heart Association
• Experience in a Home Health setting
• Driver's License
• 2 or more years of acute nursing care (hospital base) experience
• 2 or more years of Home Care experience
• Experience with OASIS

Additional Notes

• Must live more than 50 miles from the facility
• Requested time off must be submitted at profile upload and is limited to 5 days or fewer
• Candidate should be able to work independently with minimal supervision
• 1 supervisory signed reference from within the past 2 years is required
• Non-billable orientation: 8 hours
• No call, call-back, or charge responsibilities
