Registered Nurse - Hospice Case Manager

Location: Barre, Vermont
Employment Type: Contract
Contract Length: 20 weeks
Work Schedule: Monday - Friday, 36 hours per week

Job Description

• Provide hospice care in a home setting as part of hospice case management.
• Experience with hospice case management is required.
• Prior experience with rural travel is preferred.
• Experience using McKesson/Netsmart EMR systems is preferred.
• Preference for candidates without Return to Office (RTO) requests for this contract.

Required Qualifications

• Valid Registered Nurse (RN) license in Vermont.
• Minimum of 1 year of case management experience, including experience within the last year.
• Basic Life Support (BLS) certification.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Experience in a home health setting.
• Hospice care experience.
