Callback on weekends/holidays

Role Summary

Outpatient/Inpatient Nurse Case Manager supporting high-risk patients
Focuses on care transitions, discharge planning, care coordination, and utilization review
Works with culturally diverse patients across all ages, including patients with mental health needs

Required Qualifications

Associate of Science in Nursing / Associate Degree in Nursing
Valid RN license in Arizona
Case Management (3 years of experience)
Case Management experience within the last year
Experience in a(n) Outpatient setting
Experience in a(n) Inpatient setting
BLS issued by American Heart Association
Utilization Review (InterQual)

Additional Requirements

Fingerprinting must be submitted and cleared before start

Housing

Shared contractor housing only
Very limited hospital rental property
No pets or family allowed per policy
