Job Responsibilities
- Assess, plan, implement, and evaluate patient care plans in consultation with healthcare professionals.
- Provide direct bedside nursing care to pre- and post-surgery patients.
- Prepare, administer, and document prescribed medications.
- Change dressings, insert catheters, and start IVs.
- Adjust specialized medical equipment and interpret measurements.
- Monitor patient vitals and report adverse medication reactions promptly.
- Collaborate on discharge planning with the healthcare team.
- Educate patients and families on surgical procedures and post-surgery care.
- Maintain and update patient files, medical charts, and health records.
- Adhere to best nursing practices, standards, and protocols.
Required Qualifications
- Valid RN license in New York.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Minimum 1 year of experience in Home Health care, with experience within the last year.
- Experience in a Home Care setting.
- Med/Surg experience.
