Valid Speech Language Pathologist license in New Jersey
Speech Language Pathologist (1 year of experience)
Speech Language Pathologist experience within the last year
Master's degree in speech and language pathology
Experience in a(n) Home Care setting
Driver's License

Key Responsibilities

Assess patients’ appropriateness for home care and evaluate swallowing, communication, and cognitive abilities
Develop and update care plans with the patient, family, and physician
Provide treatment, education, discharge planning, and referrals as needed
Maintain documentation, regulatory compliance, and interdisciplinary communication
Travel to patient homes and facilities as needed
Use a personal vehicle for home visits
