Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 8:00 AM – 4:30 PM

Provide patient care in home health settings, including assessing patient conditions, implementing care plans, and offering education and support to patients and families.
Perform admission processes and case management duties following organizational policies and standards.
Deliver care consistent with community health practice philosophy.
Utilize thorough and timely electronic documentation and interdisciplinary communication.
Ability to travel to assigned cases.
Weekend rotations occur once every 6 weeks.
