RN Case Manager

Overview

  • Travel RN role in Case Mngmt / (917)
  • Schedule: 8:00 AM–4:30 PM with weekend rotation
  • Unit size: mainly 20 beds
  • Staffing ratio: RN 1:20

Required Qualifications

  • Valid RN license in Colorado
  • Associate of Science in Nursing / Associate Degree in Nursing
  • Case Management (1 year of experience)
  • Case Management experience within the last year
  • BLS
  • Experience with discharge planning

Preferred Qualifications

  • Discharge Planning
  • ED experience
  • Palliative Care Knowledge and GOC Conversation

Unit Details

  • Team includes CM/SW
  • EMR: Meditech / CarePort
  • Typical procedures: Discharge Planning Assessments, Discharge Planning

Work Environment

  • Day shift coverage includes Friday–Monday 8:00–4:30 and Monday–Friday 8:00–4:30
  • Equipment used: mobile, computer
