Travel RN Case Manager Position

Job Summary

Seeking a Travel Registered Nurse (RN) for Case Management in an acute care hospital setting. The role involves acute care case management focusing on Utilization Review (UR), Discharge Care Planning (DCP), and Transitions of Care including Home Health and Skilled Nursing Facility setups.

Unit Details

• Unit Size: 42-50 beds
• Patient Ratios: Approximately 2:2
• Staffing Support: Unit secretaries and techs available
• Documentation Systems: Meditech and MIDAS
• Scheduling: 8:00 AM – 4:30 PM with weekend rotation
• Floating Policy: No floating required
• Procedures: Rounding with interdisciplinary teams, physicians, and IDT (Interdisciplinary Team)
• Dress Code: Business casual

Required Qualifications

• Valid Registered Nurse (RN) license in Florida
• Minimum 2 years of Case Management experience, including experience within the last year
• Acute Hospital Case Management experience
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• Basic Life Support (BLS) certification
• Proficient in Discharge Care Planning (DCP) assessment skills
• Experience with Utilization Review using InterQual criteria
• Skilled in Discharge Planning
• Experience with MEDITECH electronic health record system
