Local candidates are not accepted within 60 miles
All requested time off must be submitted at time of application and is subject to facility needs
Facility may call off 1 shift per 2 weeks

Annual background check covering all locations lived/worked for the past 7 years
EPLS/GSA/SAM and HHS/OIG checks within 30 days of start
Physical exam
Tdap within 10 years of start or declination
TB screening: 2-step PPD or Quantiferon Gold within 90 days of the contract date; T-spot not permitted
Hepatitis B titer or vaccine
Measles, mumps, rubella, and varicella immunity documentation
10-panel drug screen within 90 days of start
COVID vaccination documentation or declination
Flu vaccination documentation or exemption during flu season
Respirator clearance for patient-facing travelers
