Required Qualifications

Valid RN license in Massachusetts
Associate of Science in Nursing / Associate Degree in Nursing
Home Health experience within the last year
1 year of Home Health experience
1 year of Certified Home Care experience
Case Management experience

Role Summary

Assess patient conditions
Implement care plans
Provide ongoing education and support to patients and families
Perform admission processes and case management responsibilities
Use timely electronic documentation and interdisciplinary communication

Onboarding / Compliance Requirements

Resume with the past 7 years of work experience
2 references within the past 4 years, including at least 1 supervisor or above in the skillset
Skills checklist completed within 1 year of submission
Primary source verification of professional license and/or certification
Background checks and registry checks required for onboarding
Fit test and respirator medical clearance if required for the skill
MMR documentation or immunity
TB screening documentation
Health statement signed by an MD, DO, NP, or PA
Education verification
SSN trace
National sex offender registry check
Exclusion list checks
Confirmation of assignment/POI form
