Minimum Requirements 
<ul><li>Doctoral Degree in Physical Therapy (DPT) from an accredited program.</li><li>Active Washington State PT license is required to start the assignment.</li><li>CPR certification required.</li><li>Home health experience required. Experience conducting SOC, evaluations, ROC, recertifications, and subsequent visits.</li><li>Experience with HCHB EMR preferred.</li><li>Ability to cover patients within a 50 mile radius from base location.</li><li>Proof of eligibility to work in the U.S.</li></ul>
 
Assignment Details
<ul><li>Facility Type: Home Health.</li><li>Shift: Days.</li><li>Shift Hours: 08:00 - 16:00 (days).</li><li>Schedule: 5x8, 40 hours per week guaranteed.</li><li>On-call/Weekend Requirement: No.</li><li>Mileage reimbursed between patient visits from first patient to last patient.</li><li>Primary responsibilities: SOC, evaluations, return-of-care visits, recertifications, and subsequent visits.</li><li>Dress code: Business casual.</li><li>May be required to float within the local coverage area as needed.</li></ul>
