Job Opportunity: Physical Therapist - Home Health

Location

  • Portland, Oregon
  • Travel/Contract position covering West Portland and surrounding areas

Job Details

  • Specialty: Therapy/Rehabilitation (Physical Therapist)
  • Shift: Day shift, 8 hours per day (5x8 hour shifts, 8:00 AM to 4:30 PM)
  • Contract Length: 13 weeks
  • Patient Ratio: 1:1, with up to 6 patient visits per day expected
  • Productivity Standard: 30 productivity points per day
  • Weekend Work: Not required
  • On-Call: Not required
  • Floating: Not required
  • Block Scheduling: Not accommodated
  • Locals accepted within 50-75 mile radius
  • Pending license accepted
  • Guaranteed hours with possibility of being called off up to one shift every two weeks
  • Transportation: Must have own reliable transportation; daily travel radius up to approximately 60 miles (60-90 minutes total drive time)
  • Attire: Business casual preferred; scrubs allowed but no specific color required; no jeans
  • Charting System: EPIC Remote Client (laptop) and Rover app (phone)
  • Parking: Free
  • Mileage Reimbursement: IRS standard rates

Required Qualifications

  • Valid Physical Therapist license in Oregon (pending license accepted)
  • Minimum 1 year of Physical Therapist experience within the last year
  • Experience in a Home Health setting
  • Basic Life Support (BLS) certification issued by American Heart Association or Heart & Stroke Foundation
  • Must have own transportation

Experience

  • 1 year of skilled home health experience required
  • First-time travel therapists accepted
  • No prior travel experience required

Compliance and Documentation Requirements

  • Government-issued photo ID (Driver’s License or State ID)
  • Social Security Card (as applicable)
  • Background check within 90 days covering all locations lived/worked in past 7 years, including SSN trace, death master index, nationwide sex offender, education verification, motor vehicle record (required for home health), and state-specific abuse/neglect searches as applicable
  • Education verification required
  • Employment application covering at least last 5 years of employment
  • Minimum of 2 professional references covering 1 year of employment in a comparable specialty/setting
  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness for work and infectious disease status
  • Immunizations and health screenings including:
    • Tetanus/Diphtheria/Tdap (within 10 years, declination accepted)
    • Negative TB test or chest X-ray within 12 months (annual updates required)
    • MMR (Measles, Mumps, Rubella) vaccination or positive titer
    • Varicella vaccination or positive titer
    • Hepatitis B vaccination or signed declination
    • Seasonal flu vaccine during flu season or signed declination
    • COVID-19 vaccination record preferred; declinations accepted with adherence to masking policies
  • Drug screening: 9-panel drug screen within 12 months (instant acceptable)
  • Respiratory safety clearance required annually for travel candidates
  • Annual regulatory and specialty-specific training modules (not required for home health at this facility)
  • Photo for badge (passport style with neutral background) required

Additional Notes

  • Facility cannot accommodate alternative shift patterns such as 4x10 or 3x12
  • Facility groups patient visits to minimize driving time
  • No floating between units required
  • Must comply with facility’s float policy acknowledgment if applicable
  • Some state-specific background and license verifications apply, especially for Oregon and Washington border placements
  • Candidates must complete required documentation and certifications prior to start date
