Job Opportunity: Home Health Registered Nurse (RN)

Job Summary

• Provide skilled nursing care to patients in their homes, focusing on geriatric populations.
• Perform comprehensive patient assessments and collaborate with interdisciplinary teams to develop and oversee individualized Plans of Care.
• Responsible for in-home patient care, interdisciplinary care coordination, and certifying providers.
• Charting required using Homecare Homebase (HCHB) and OASIS systems, completed while at patient homes.
• Average caseload: 4-6 patients per day.
• Must participate in an on-call rotation (including one night of call during the week).
• Work schedule: Monday to Friday, 8:00 AM to 5:00 PM, with weekends as needed.
• Must be willing to travel up to 50 miles from home base to patient locations.

Required Qualifications

• Minimum 2 years clinical experience overall, with at least 1 year in Home Health nursing.
• Valid Registered Nurse (RN) license in hand at the time of assignment (no pending licenses accepted).
• BLS certification required. ACLS and PALS certifications preferred or required depending on unit.
• Experience with Homecare Homebase (HCHB) and OASIS charting systems preferred (1 year preferred).
• Non-smoker preferred due to patient sensitivity (patients with COPD).
• Must be open to on-call duties and weekend shifts as needed.

Additional Requirements & Compliance

• Current driver’s license/state ID required.
• Background checks required within 1 year, including seven years of residency/work history, social security trace, county searches, sex offender registry, and state driver’s record.
• Annual federal exclusions and sanctions checks (EPLS/GSA/SAM, HHS/OIG).
• Physical exam within 12 months confirming good health.
• Negative TB test or chest X-ray within required timeframes; annual TB questionnaire.
• Immunization or proof of immunity for Measles, Mumps, Rubella, Varicella, and Hepatitis B declination required.
• 9-panel drug screen within 12 months.
• Seasonal flu vaccination or declination required during flu season (Oct 1 - Mar 31).
• Auto insurance required if using personal vehicle for work.
• Annual competency exams including medication/pharmacology and infection control/bloodborne pathogens.
• Annual skills checklist for unit-specific competencies.
• Professional references required within 3 years:

  • 2 for travel, allied, or per diem acute care roles
  • 1 for per diem non-acute care roles

Work Environment & Other Details

• Dress code: Scrubs, any color.
• Must be able to hit the ground running and meet productivity expectations (30 points per week, equivalent to 40 hours).
• Local candidates accepted within a 50-mile radius.
• Mileage reimbursed at IRS standard rates.
