Job Overview

• Shift: Day, Monday–Friday, 8:00 AM–4:00 PM
• Weekly hours: 40
• Schedule note: 1 Saturday coverage per month

Required Qualifications

• Valid Occupational Therapist license in Oklahoma
• BLS issued by American Heart Association or American Red Cross
• 2 professional references within the past 3 years

Additional Requirements

• Government-issued photo ID
• E-Verify or attestation
• Background check with all locations lived/worked in the past 7 years; must be cleared before start and updated annually
• EPLS/GSA/SAM check
• HHS/OIG check
• Skills checklist
• Employment application
• Competency exam
• Medication/pharmacology competency exam, as applicable
• HIPAA acknowledgment
• Physical exam within 1 year of start
• Tdap vaccination within 10 years of start, or declination
• TB screening within 12 months of start
• Measles, mumps, rubella, varicella, and hepatitis B immunity/vaccination documentation
• Drug screen within 1 year of start
• Current COVID vaccination or COVID declination form
• Flu documentation or declination, if applicable
• Review and sign required orientation, confidentiality, and facility policy documents before start
• Passport-style photo for badge

Notes

• HealthStream modules must be completed before the Monday start date, by Friday afternoon
• First 16 hours of orientation are non-billable
