Operating Room Registered Nurse (Travel/Contract)

Location: Phoenix, Arizona
Shift: Day (4x10 hours), 6:30 AM – 5:00 PM
Contract Length: 13 weeks

Requirements / Qualifications

• Valid RN license in Arizona
• Minimum 2 years of Operating Room (OR) experience, including experience within the last year
• ACLS (Advanced Cardiac Life Support) certification
• BLS (Basic Life Support) certification issued by the American Heart Association
• Electronic Fetal Monitoring (EFM) certification
• National Institutes of Health Stroke Scale (NIHSS) certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Certifications Required

• ACLS (Advanced Cardiac Life Support)
• BLS (Basic Life Support)

Preferred Skills

• Previous charge nurse experience
• Neurology
• Burns
• Scrub/hand/burn skills

On-Call Requirement

• On-call availability required; details discussed during interview

Licensure

• Current, unencumbered RN license valid in Arizona
• Primary source verification required within 15 days prior to start

Health and Compliance Requirements

• Physical exam within 1 year of start, completed within 72 hours of contract signing
• Fit mask test within 1 year, updated annually (can be completed onsite)
• Immunizations and titers required within 72 hours of contract signing:

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Negative TB test (Qgold or Tspot within 1 year) or chest X-ray if prior positive
  • Rubeola (Measles) immunity or vaccination proof
  • Mumps immunity or vaccination proof
  • Rubella immunity or vaccination proof
  • Hepatitis B vaccination or declination
  • Varicella immunity or vaccination proof
• Negative drug test within 30 days of start (specific drug panels required)
• Seasonal flu vaccination or approved medical/religious exemption during flu season (Oct 1 – Mar 31)

Background and Verification Requirements

• Background check within 1 year of start, including SS trace, 7-year county, national criminal, and FACIS III
• Employment application with last 7 years of employment history and explanations for gaps over 30 days
• Work history verification for past 3 years
• Education verification (diploma or transcripts accepted)
• References required per agency policy (not facility requirement)

• First-time travel nurses are welcome to apply
• Local candidates are also considered
• Candidate must have a current and valid government-issued photo ID (driver’s license, state ID, or passport)
• Candidates must complete facility-specific orientation acknowledgments and confidentiality forms prior to starting
• Candidates must watch and upload proof of completion of the facility’s New Employee Orientation video before starting

This position offers a challenging and diverse surgical environment with a focus on trauma and general OR cases, suitable for RNs with solid perioperative experience and required certifications.
