Provide nursing care in a home health setting.
Assess patient conditions, implement care plans, and provide education and support to patients and families.
Perform admission processes and case management duties.
Deliver patient care following community health practice standards and organizational policies.
Document thoroughly and communicate within an interdisciplinary team.
Weekend rotations occur once every 6 weeks.
Ability to travel to assigned cases is required.

Motor Vehicle Record (MVR) check within 45 days of start.
Fit test for N95 respirator within 1 year, updated annually (no exemptions allowed).
