Summary

This role focuses on wound, ostomy, continence, and complex wound care. Responsibilities include patient assessment, care planning, treatment evaluation, staff education, quality improvement, and collaboration with the care team.

Required Qualifications

• Associate degree in nursing from an accredited nursing program
• New York State RN license
• Primary source verification of RN license and registration from NYSED
• AHA BLS
• Current certification in one of the following:

  • CWOCN
  • CWON
  • CWCN
  • CWS
  • WCC
• 2 years of Med/Surg experience
• Wound and colostomy experience

Key Responsibilities

• Conduct wound-focused health assessments and physical assessments
• Develop, revise, and evaluate standards of care and plans of care
• Assess and document patient response to wound, ostomy, continence, fistula, and complex wound treatment
• Provide patient, family, and staff education
• Participate in quality improvement activities
• Collaborate with the health care team and make referrals as needed
• Manage rented specialty equipment such as specialty beds and negative pressure devices
