Registered Nurse - LTAC

Location: Hazard, Kentucky

Job Description

• Minimum 2 years of current LTAC experience required.
• Experience in Medical-Surgical, Telemetry, or ICU settings preferred.
• Patient ratio: 1:4.
• Must be proficient with Meditech EMR.
• Every other weekend shift required.
• Blocked scheduling cannot be accommodated.
• Schedule typically follows a 36/48 hour rotation over a two-week pay period.
• Orientation occurs bi-weekly.
• Candidates must reside within 75 miles of the facility.

Required Qualifications

• Valid RN license in Kentucky.
• Minimum 2 years of Long Term Acute Care (LTAC) experience, including experience within the last year.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• American Heart Association Basic Life Support (BLS) certification.
• American Heart Association Advanced Cardiac Life Support (ACLS) certification.
• NIH Stroke Scale (NIHSS) certification.
• Orientation Safety Manual Acknowledgement.

Certifications & Competencies

• BLS and ACLS (AHA Instructor/Provider/RQI accepted).
• NIH Stroke Certification (required for all RN specialties except OR and Psych).
• Medication/Pharmacology Competency Exam.
• Dysrhythmia/EKG Competency Exam (required except for Psych, OR, Rehab, and Med Surg Allied positions).
• Core Competency Parts I, II, and III.
• Orientation Safety Manual Acknowledgement.

Health & Compliance Requirements

• Annual physical exam or physician’s note within 1 year prior to start, stating clearance to work without restrictions.
• Fit mask testing completed onsite (PAPR not accommodated; exemptions reviewed onsite).
• Immunizations and/or titers required prior to start:

  • Measles (Rubeola), Mumps, Rubella (proof of 2 MMR vaccinations or positive titer).
  • Varicella (proof of 2 vaccinations or positive titer).
  • Hepatitis B series, titer, or declination (declination must be within 1 year).
  • Tetanus/Diphtheria/Tdap (Td not accepted; valid for 10 years).
• Negative TB test via QuantiFERON blood test within 1 year prior to start (T-Spot/PPD not accepted). If positive, chest X-ray within last 5 years plus TB questionnaire required.
• Drug testing through a specified 9-panel bundle (no THC) with expanded opiates and oxycodone, within 30 days of start; annual updates required.
• Seasonal flu vaccination or approved medical/religious exemption required during flu season.

Background & Verification

• Comprehensive background check including national and county searches, sex offender, fugitive, violent offender databases, and social security verification.
• Education verification included in background or third-party verification.
• Employment application and resume with education and work history required.
• Work history verification for the past 7 years through a third-party vendor; attestation letters accepted if verification attempts are exhausted.
• Two professional references within 1 year of start date required (one must be a supervisor or higher; Charge RN acceptable for one reference).

Scheduling & Other Notes

• Every other weekend required.
• Blocked scheduling is not possible.
• Candidates must submit assignment confirmation within 24 hours of offer acceptance.
• Only one job submission allowed at a time.

Additional State-Specific Requirements

• Kentucky Vulnerable Adult Maltreatment Registry verification within 30 days prior to start; annual update required.

This position is ideal for experienced LTAC RNs with strong critical care skills and the ability to work a consistent schedule in a travel contract setting.
