Case Manager (Registered Nurse) - Travel/Contract
Location: Oakland, California
Contract Length: 13 weeks
Shift: Day (5x8 hours), 8:00 AM – 4:30 PM
Hours per Week: 40
Job Summary
- Provide care coordination and care transitions planning throughout the acute care patient experience.
- Collaborate with physicians, utilization managers, social workers, bedside nurses, and interdisciplinary teams to ensure timely patient progression and appropriate level of care transitions.
- Focus on preventing unnecessary admissions or readmissions by managing complex clinical and social situations efficiently.
- Lead and supervise healthcare teams in developing cost-effective care coordination plans.
- Participate actively in utilization management, including reviewing medical records for level of care compliance and expediting transition planning.
- Maintain compliance with CMS, Joint Commission, and other regulatory requirements related to transitions of care.
- Identify barriers to care progression and work with multidisciplinary teams to resolve them.
- Communicate with financial counselors regarding uninsured or underinsured patients and make appropriate referrals.
- Participate in department operations, contribute to process improvements, and maintain professional relationships with healthcare teams and community agencies.
Required Qualifications
- Valid Registered Nurse (RN) license in California.
- Minimum 5 years of acute care case management experience, including inpatient and acute care settings.
- Acute care case management experience within the last year.
- Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Job Duties Highlights
- Conduct initial and ongoing patient assessments including medical, psychosocial, and financial information gathering.
- Screen for ancillary supportive services such as palliative care.
- Oversee case management assistants in determining post-acute care preferences.
- Monitor length of stay and collaborate on utilization management documentation.
- Ensure delivery of Medicare Important Message within required timeframes.
- Participate in patient rounds and interdisciplinary team meetings.
- Develop and communicate transition plans involving patients, families, payers, and community resources.
- Identify and manage high utilizers of emergency services and patients with complex psychosocial needs.
- Maintain thorough documentation in electronic health records and other systems.
- Follow policies for physician advisor referrals and escalation processes.
Additional Requirements
- Must review and sign facility-specific documents including job description, standards of business conduct, confidentiality agreements, HIPAA agreements, and orientation materials prior to start.
- Complete required facility-specific competency forms and attestations as applicable.
- Comply with all regulatory and facility-specific training and documentation requirements.
