Job Opportunity: RN Home Health

Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks

Required Experience & Qualifications

• Minimum 1 year Certified Home Care experience
• Minimum 1 year Case Management experience
• Current RN licensure in Massachusetts
• Reliable, independent transportation for home visits
• Willingness to work weekends, holidays, and be on call as needed (weekend rotation every 6 weeks)

Prior travel experience: Not required

Job Responsibilities

• Provide clinical services to patients of all ages in their homes
• Assess patient conditions and implement care plans
• Provide ongoing education and support to patients and families
• Perform admissions and case management according to organizational policies
• Maintain thorough and timely electronic documentation and interdisciplinary communication

Credential & Compliance Requirements

• Professional license verification (primary source, no conditional licenses)
• Submission of a recent professional photo (no selfies, plain background)
• Motor Vehicle Record (MVR) check within 45 days of start
• N95 Fit Test within 1 year, updated annually
• Nurse Aide Registry check within 30 days of start (required for all employees)
• Immunizations and health screenings including:

  • MMR (measles, mumps, rubella) vaccination or proof of immunity
  • Influenza vaccine during flu season (no exemptions)
  • TB test or blood test within 3 months of start, updated if >6 month break
  • Hepatitis B vaccination strongly preferred
  • Tdap vaccine preferred (within 10 years)
• Physician health statement within 1 year of start, updated if >6 month break
• Criminal background check, CORI, SSN trace, national sex offender check, and exclusion checks within 45 days of start and updated as required
• Submission of resume with 7 years of work history including city and state
• Two recent professional references (minimum one supervisor or above)
• Skills checklist and candidate submission coversheet required for submission
• Respirator Medical Clearance attestation if fit test required

Additional Notes

• Must be able to work independently and reliably in a home care setting
• Weekend rotations occur approximately once every 6 weeks
• All documentation and credentialing must be completed and verified prior to onboarding
