Required Qualifications

ARRT
BCLS
BLS
Valid driver’s license or state ID
Primary source verification of professional license and/or certification
COVID-19 vaccine series or bivalent booster
MMR immunization or positive titer
Fit test
Hepatitis B vaccination series or declination
Influenza vaccine
2-step PPD or IGRA (Quantiferon/Tspot)
Varicella immunization or positive titer
Tdap immunization
CNE profile
2 professional references, including at least 1 supervisory reference
Criminal background check
First Look orientation packet
Non-Employee confidentiality agreement
Employment verification for the last 5 years
Covid and Flu Policy CNE Form
Confirmation of Assignment
