Physical Therapist - Home Health
Location: Tampa, Florida
Employment Type: Travel/Contract
Contract Length: 20 weeks
Job Description
- Provide skilled home health physical therapy services including assessment, cardiopulmonary/circulatory care, ortho, neuro, and patient/family education.
- Utilize equipment and devices such as gait belts, rolling walkers, sit-to-stand lifts, sliding boards, collars, supplemental oxygen, prosthetics, and wheelchairs.
- Manage home exercise programs and medication education.
- Recognize signs and symptoms of physical, sexual, and psychological abuse.
- Ensure safety and injury prevention during patient care.
- Perform documentation and billing related to home health visits.
- Patient population includes med/surg, cardiac, ortho, post-surgical, COPD, CHF, pneumonia, stroke, Parkinson’s, trauma, amputees, and others requiring ADL assistance.
- Conduct 6-8 home visits per day, with a minimum of 6 points per day (points system at manager’s discretion).
- Self-schedule visits and hours based on office needs.
- Weekend work required 1-2 weekends per contract.
Required Qualifications
- Valid Physical Therapist license in Florida.
- Minimum 1 year of Physical Therapist experience, including experience within the last year.
- Experience in a Home Health setting.
- Basic Life Support (BLS) certification.
Work Schedule
- Day shift, Monday through Friday, 8:00 AM to 5:00 PM (5 days x 8 hours = 40 hours/week).
Compliance and Documentation Requirements
- Background checks including 7-year history, education verification, EPLS/GSA/SAM, HHS/OIG, and Florida AHCA Level II fingerprint clearance required prior to start.
- Foreign equivalency evaluation required if education was completed outside the U.S. or Puerto Rico.
- Physical exam within 6 months of start date.
- Negative TB test (PPD, Qgold, or Tspot) within 10 weeks of start; chest X-ray and TB questionnaire if prior positive PPD.
- Annual TB questionnaire completion required in lieu of annual TB test.
- Immunizations or titers required for Measles (Rubeola), Mumps, Rubella, Varicella, and Hepatitis B (titer, series, or declination accepted).
- Standard 9-panel drug test within 30 days of start (instant tests not accepted).
- Respirator medical clearance if respirator use is required.
- Photo for badge with solid background required prior to start.
- Personal or auto insurance must be current and active during assignment.
- Completion of facility-specific online training modules prior to start (approx. 4-8 hours).
- References: Two professional references from the past 2 years required.
- Employment application and skills checklist must be submitted and approved.
Additional Information
- Scrubs: Any solid color scrub allowed.
- Equipment provided includes laptop, MiFi for internet access, and portable printers.
- Documentation systems used include proprietary scheduling and timesheet software.
- Candidates must comply with facility dress code, tobacco policy, and mask requirements.
- Candidates must complete medication and competency exams as applicable.
- Stroke education CEUs and NIH Stroke Certification may be required depending on patient population.
