Registered Nurse – Med/Surg Telemetry

Job Description

• Provide nursing care on Med/Surg Telemetry unit with potential to float to related units including Emergency Department.
• Rotating shifts: 7:00 PM – 7:30 AM (night) and 7:00 AM – 7:30 PM (day), typically 3 shifts per week (12-hour shifts).
• Shift assignments may vary to meet operational needs; facility will attempt to honor preferred shifts but reserves the right to rotate.

Required Qualifications

• Valid RN license in Ohio.
• Associate of Science in Nursing / Associate Degree in Nursing.
• Minimum 1.5 years of Med Surg / Telemetry experience.
• Med Surg / Telemetry experience within the last year.
• Current certifications required:

  • ACLS (Advanced Cardiac Life Support)
  • BLS (Basic Life Support) issued by American Heart Association or American Red Cross
  • NIHSS (National Institutes of Health Stroke Scale)

Required Documentation & Compliance

• Government-issued photo ID (e.g., driver’s license) required and must be carried on assignment.
• Background checks including:

  • FCRA-compliant 7-year criminal history search
  • Monthly EPLS/GSA/SAM and HHS/OIG checks
  • FBI and BCI fingerprinting within 12 months, updated annually
• Education verification required.
• Physical exam within 1 year of start confirming ability to perform essential job functions.
• Fit mask test (N95) within 12 months, updated annually.
• Immunizations and health screenings required:

  • Tdap vaccine every 10 years (no declinations accepted)
  • Negative TB test or chest X-ray as applicable, updated annually
  • MMR (Measles, Mumps, Rubella) vaccination or positive titers with boosters if needed
  • Hepatitis B vaccination/titer or declination
  • Varicella vaccination or positive titer with booster if needed
  • Annual flu vaccination (no declinations accepted)
• Drug screening: Standard 10-panel drug screen within 12 months, updated annually (instant tests not accepted).
• Eye exam (Snellen) within 1 year of start.
• Two professional references from employment within the past year (manager, supervisor, charge nurse, or director).

Orientation & Training

• Completion of facility-specific online onboarding modules prior to start; must be done on a computer using Google Chrome.
• Modules include training videos and tests; skipping videos is not allowed.
• Medication administration and ECG competency exams are required at orientation with minimum passing scores (SafeMedicate 90%, ECG 80%).
• Candidates with less than 6 months gap from previous assignment at the facility may be exempt from some orientation testing.
• SafeMedicate prep materials provided before orientation.

Additional Notes

• Candidates must have legal name consistency across all documents (license, ID, work order).
• Fingerprinting must be completed both through the agency and at the facility.
• Candidates with latex allergies must notify the facility in advance.
• Religious and medical exemptions for vaccines and fit mask are accepted but require agency approval and facility review.
• Candidates must be prepared to comply with all facility policies and complete all required documentation timely to avoid delays in start date.

Work Hours

• 36 hours per week, typically 3 shifts of 12 hours each.
• Rotating day and night shifts.
