Overview

• Location: Port Angeles, Washington
• Schedule: Full-time, 40 hours/week, 5x8s
• Shift: Day shift, weekends/holidays
• Contract length: 13 weeks

Role Summary

Provides nursing visits in patients’ homes as part of the interdisciplinary care team, supporting continuity of care and coordinating with the case manager RN.

Required Qualifications

• Associate’s degree
• At least 1 year of recent experience in home health, acute care, or long-term care
• Current Washington State Registered Nurse license
• Current BLS certification
• Valid Washington driver’s license
• Proof of insurance for the automobile used for work
• Reliable automobile transportation to travel to patient residences
• Driving record that meets current employer standards
• Cell phone with activated voicemail
• Computer internet connectivity

• Temporary/traveler assignment
