• Pharmacist experience within the last year

Required Skills / Experience

• Review patient records to determine appropriateness of medication therapy
• Evaluate patient condition to ensure issues are being treated
• Develop medication plans that minimize risk of adverse side effects
• Consult with prescribers, nurses, and patients on medications and doses
• Advise on correct administration of drugs
• Assess results and outcomes of pharmaceutical treatments
• Collaborate with healthcare professionals to ensure optimal patient care
• Keep accurate documentation of medication plans and patient progress
• Routinely consult with physicians and nurses regarding orders, medication availability, side effects, incompatibilities, formulary considerations, and current care path management
• Participate in education of students and residents as needed
