Required Qualifications
- Valid Occupational Therapist license in California
- Bachelor's degree
- Driver's License

- Competency exam
- Physical exam within 12 months of start
- Negative TB test or chest X-ray within 1 year of start
- TB questionnaire if TB positive
- Measles, mumps, rubella, hepatitis B, varicella, and seasonal flu documentation or vaccination requirements
- Drug screening within 12 months of start
- 2 professional references within 3 years of hire
- Skills checklist
- Signed orientation safety manual acknowledgment
