Experience Requirements

- State License Verification (Primary Source Verification within 30 days of start)
- NURSYS Verification for RN within 30 days of start
- Orientation Safety Manual Acknowledgement (required)
- NIH Stroke Certification (as required by unit, updated annually)
Required Background Checks & Verifications
- 7-year criminal background check (including felony checks) within 3 months of start
- SSN Trace, National Criminal, National Sex Offender checks
- Education verification from primary source (hard copies not accepted)
- EPLS/GSA/SAM check within 3 months of start and monthly thereafter
- HHS/OIG check within 3 months of start and monthly thereafter
- Foreign equivalency education evaluation if applicable
- Transcript/Diploma if no education verification on file
- Employment application and work history verification (minimum 2 past employments verified)
- References (2 within 3 years) required for Aya candidates
Health & Compliance Requirements
Candidates must complete all health-related requirements and lab work within 5 days of signing the contract:
- Physical exam within 1 year of start (updated annually)
- Fit mask test within 1 year of start (specific acceptable mask types)
- Negative TB test or chest X-ray within 1 year of start (with follow-up requirements for positive results)
- TB Questionnaire (acceptable in lieu of annual TB test renewal)
- Immunizations or positive titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (Surface AB positive titer, completed series or declination)
- Varicella
- Drug screening within 30 days of start (includes amphetamines, cocaine, fentanyl, heroin, opiates, etc.)
- Seasonal flu vaccination or signed declination (required during flu season)
- Eye exam (Snellen) within 1 year of start
- COVID vaccination copy preferred but not required

- Medication Test/Pharmacology Competency Exam (as required)
- Color Blind Testing (required once)

- Photo for badge (passport-style with solid background)

Summary
