Job Details

Required Background Checks & Verifications

  • 7-year criminal background check (including felony checks) within 3 months of start
  • SSN Trace, National Criminal, National Sex Offender checks
  • Education verification from primary source (hard copies not accepted)
  • EPLS/GSA/SAM check within 3 months of start and monthly thereafter
  • HHS/OIG check within 3 months of start and monthly thereafter
  • Foreign equivalency education evaluation if applicable
  • Transcript/Diploma if no education verification on file
  • Employment application and work history verification (minimum 2 past employments verified)
  • References (2 within 3 years) required for Aya candidates

Health & Compliance Requirements

Candidates must complete all health-related requirements and lab work within 5 days of signing the contract:

  • Physical exam within 1 year of start (updated annually)
  • Fit mask test within 1 year of start (specific acceptable mask types)
  • Negative TB test or chest X-ray within 1 year of start (with follow-up requirements for positive results)
  • TB Questionnaire (acceptable in lieu of annual TB test renewal)
  • Immunizations or positive titers for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B (Surface AB positive titer, completed series or declination)
    • Varicella
  • Drug screening within 30 days of start (includes amphetamines, cocaine, fentanyl, heroin, opiates, etc.)
  • Seasonal flu vaccination or signed declination (required during flu season)
  • Eye exam (Snellen) within 1 year of start
  • COVID vaccination copy preferred but not required

Additional Competencies & Documents

  • Medication Test/Pharmacology Competency Exam (as required)
  • Color Blind Testing (required once)
  • Competency Exam (as required by specialty)
  • Photo for badge (passport-style with solid background)

State-Specific Requirements (New York)

  • NYS Nurse Aide Registry (if applicable) within 3 months of start for certain care areas
  • NYS Criminal Background Check Fingerprint Results (for specific units)
  • NYS Disciplinary Action Search (for specific units)
  • NYS Sex Offender Registry Search within 30 days of start
  • NYS Medicaid Exclusion List check within 3 months of start and monthly thereafter
